How to create a feedback webpage?

Please tell me how to create a page like that:
- Header: "Here you can give your opinions and suggestions about the work of the website.

- The window in which the text is typed.

- Button: "Submit"

First, decide how it should work for you:
- a separate block on the page
- a pop-up window
- with or without page reload

Then what the website as a whole works on - CMS, php, html.

If a CMS is used, then the easiest way is to find a plugin for your own.
For WP - proven over the years https://wordpress.org/plugins/contact-form-7/

WAO

THE FEEDBACK FORM SHOULD CONTAIN A MINIMUM OF REQUIRED FIELDS.
You should not collect more information from the client than you need in order to contact him.
Any additional field is a minus in the conversion of final call requests, and hence sales. All the information can be collected later, when you can interest the client personally in your trading offer.
You can easily set the necessary and optional fields to fill in, as well as the phone mask, validation of the entered data with hints for correct filling in in any feedback form builder for the site.

DO NOT CONTAIN COMPLEX CAPTCHAS AND AGGRESSIVE ANTISPAM SYSTEMS.
No one will look for all the traffic lights and motorcyclists in the picture for a long time, if they are not there, or try switching the keyboard layout to enter meaningless text once again.
Think 10 times, do you need an anti-spam system?
Is this problem so urgent for you? Are there any other solutions, for example, a block on the IP of spammers, or a limit on sending a form, no more than one submission per hour from one IP and from one browser. Such functionality for blocking unwanted applications contains any more or less serious service for creating feedback forms for the site.

THE FEEDBACK FORM SHOULD BE IN A PROMINENT PLACE ON MOST PAGES.
The form should always be visible, or intuitively indicate on the page where it can be found. It can also open as a separate window on top of the page content, reacting to certain user actions. For example, moving the mouse to close the page, scrolling to the end of the page, or a certain time that the visitor spent on the site, or the depth of views, etc.

THE FEEDBACK FORM ON THE WEBSITE MUST CORRECTLY SEND THE RECEIVED DATA TO THE CRM OR IN THE FORM OF A LETTER TO THE MAIL.
The received applications should be sent to the mail or CRM in a convenient structured form and in the correct encoding.
The application must contain additional parameters that will allow you to process or filter the application correctly and conduct analytics: the page of filling out the application, the keyword of the transition from the search engine, ip address, country and city, utm tags.

TO LOOK HARMONIOUSLY AND FIT INTO THE STYLE OF PRESENTATION OF INFORMATION AND THE DESIGN OF THE SITE.
The form should not look outdated by design or specially embedded in the page. It should be perceived as an integral part of the site structure. It is especially important to work correctly in the mobile adaptive version of the form, as well as in any browser, and on any model of mobile device.

IT IS EASY TO INSTALL AND WORK CORRECTLY ON POPULAR CMS (JOOMLA, OPENCART, WORDPRESS, DRUPAL) IF YOU USE A READY-MADE FEEDBACK FORM.
The feedback form should not conflict with the CMS on which your site is running. Call requests must be received correctly from the form and not blocked by the engine. The form should not weigh down the page and lengthen the time required to load the page.
Most feedback form designers work correctly on popular CMS.

DO NOT INTERFERE WITH VIEWING INFORMATION ON THE SITE AND DO NOT CLOSE IMPORTANT ELEMENTS ON THE PAGE.
If the feedback form, when activated, closes products, important text on the page, a basket of goods and other important elements, and at the same time it is unclear how to close it, then such a form should be finalized.
The form should be an addition to the page, but in no way should the page be an addition to the form.

CONTAIN A LINK TO THE PERSONAL DATA PROCESSING POLICY.
Many neglect this function, for the sake of conversion, or simply do not want to spend time on it: creating text, its layout, etc. To do it or not is up to everyone to decide.

CONTAIN A CLEAR AND SHORT CALL THAT WOULD EXPLAIN WHAT THE VISITOR WILL RECEIVE BY FILLING OUT THE FEEDBACK FORM.
The visitor must understand exactly what he will get by filling out the form. A discount, a gift, will participate in the promotion or just get a call back within an hour.

Don't make the form text too long. Ideally, it should contain only a bright title, a subtitle that would reveal the idea of the title and a bright button that would call for sending data. It will be great if you thank the visitor at the end and give him additional information after submitting the form.

BE FLEXIBLE.
The form should be able not only to collect applications for a callback, but also to be configured to conduct simple polls, votes, be able to accept files, for example, copies of documents.
